The PROGRAM FORM is for performers in the 2017 San Francisco Fringe Festival. Please complete this form and SUBMIT by MARCH 3, 2017. This information will be used to create your page on the Fringe mobile app (“San Francisco Fringe Fest” in the Apple App Store and on Google Play), the Fringe website (www.sffringe.org), and on your advance ticket sales page. You will have the opportunity later to add more information including bios of cast and crew.

Review your title CAREFULLY including spelling and capitalization. EVERYTHING in the SF Fringe is organized by your title and there is a $150 fee for any changes to the title.

You will have the opportunity to add more information later, including bios for cast and crew.
